- Restrictions: You may request limits on how we use or share your PHI. While we are not required to agree in all cases, we will comply when legally required (e.g., if you pay in full for services and request we not share with insurance).
- Confidential Communication: You may request that we contact you at a specific phone number, email, or mailing address.
- Paper Copy: You have the right to request a paper copy of this Notice at any time.
- Breach Notification: You have the right to be notified if your PHI is compromised in a data breach.
Our Legal Duty
Velara Wellness Nursing Inc. is required by law to:
- Protect the privacy and security of your PHI
- Provide you with this Notice outlining our duties and practices
- Abide by the terms of this Notice currently in effect
We may update this Notice at any time to reflect changes in our policies or the law. Updated versions will be posted on our website and available upon request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with:
- The U.S. Department of Health and Human Services (HHS), or
- Directly with our office at Velara Wellness Nursing Inc.
You will not face retaliation for filing a complaint.
